The CCC Plus Waiver (Medicaid home and community care)

Virginia's Medicaid route to help at home -- not a way to pay for assisted living.

Quick answer

The Commonwealth Coordinated Care Plus Waiver is a Medicaid home and community-based services waiver that covers personal care (agency or consumer-directed), respite (agency or consumer-directed), adult day health care, personal emergency response systems with medication monitoring, assistive technology, environmental modifications, private duty nursing, transition services and services facilitation.

What does the CCC Plus Waiver cover in Virginia?

The Commonwealth Coordinated Care Plus Waiver is a Medicaid home and community-based services waiver that covers personal care (agency or consumer-directed), respite (agency or consumer-directed), adult day health care, personal emergency response systems with medication monitoring, assistive technology, environmental modifications, private duty nursing, transition services and services facilitation.

Personal care has a soft cap of 56 hours a week (exceptions possible); respite is limited to 480 hours per state fiscal year; environmental modifications and assistive technology up to $5,000 each per calendar year; private duty nursing up to 112 hours a week; transition services up to $5,000 per lifetime.

The CCC Plus Waiver pays for services at home and in the community. It does not pay assisted living room, board or care; in Virginia the Auxiliary Grant is the public program tied to licensed assisted living.

Who qualifies for the CCC Plus Waiver in Richmond?

A person qualifies when they are financially eligible for Medicaid and a screening finds they meet nursing facility level of care -- functionally dependent with medical or nursing needs -- and that waiver services can safely keep them at home; older adults, people with physical disabilities and people who need ongoing skilled nursing are the groups served.

Before nursing facility Medicaid or CCC Plus Waiver services, a person needs a Long-Term Services and Supports (LTSS) Screening, requested through the local department of social services and done by a community-based team (a social worker and a health department nurse) or, during a hospital stay, by the discharge planner.

Members are enrolled in Cardinal Care, Virginia's Medicaid program, usually through a managed care plan whose care coordinator then helps arrange services.

How do you apply for the CCC Plus Waiver?

Ask the local Department of Social Services for a Long-Term Services and Supports screening and apply for Medicaid at the same time -- online at CommonHelp, by phone through Cover Virginia, or on paper with Appendix D -- then choose providers once both are approved.

Apply online at CommonHelp.virginia.gov, by phone through Cover Virginia at 1-833-522-5582 (TDD 1-888-221-1590), or on paper through the local department of social services, requesting Appendix D for long-term services and supports.

If the person is in a Richmond hospital, ask the discharge planner to do the screening before discharge; it can save weeks.

PACE (Program of All-Inclusive Care for the Elderly) serves adults 55 and older who meet nursing facility level of care and can live safely in the community.
