Observation Status at Richmond Hospitals: Why Two Words Decide Who Pays for Rehab

A parent can spend nights in a VCU, Bon Secours or HCA hospital bed without being admitted. Here is why that matters for Medicare rehab coverage and what to ask.

Quick answer

Observation status means a patient is in the hospital as an outpatient while doctors decide whether to admit them. The patient may be in a regular room, wear a hospital gown and stay more than one night, but for Medicare purposes they have not been admitted as an inpatient.

TL;DR

Medicare covers skilled nursing facility rehab only after a qualifying inpatient hospital stay; nights spent under observation status do not count. At any Richmond-area hospital, ask in writing whether your parent is an inpatient or under observation, and ask early.

What is observation status?

Observation status means a patient is in the hospital as an outpatient while doctors decide whether to admit them. The patient may be in a regular room, wear a hospital gown and stay more than one night, but for Medicare purposes they have not been admitted as an inpatient.

It is common after falls, chest pain and dizziness, exactly the episodes that often end with an older adult needing a few weeks of rehab before going home.

Why does observation status matter for rehab after a Richmond hospital stay?

Medicare Part A pays for skilled nursing facility care, the short-term rehab many people need after a hospital stay, only when it follows a qualifying inpatient admission and the person needs daily skilled nursing or therapy. Time under observation does not count toward that inpatient requirement.

So two patients with the same broken wrist and the same three nights at the same hospital can face very different bills for rehab, depending on a status decision most families never hear about.

What should families ask at VCU, Bon Secours or HCA hospitals?

Ask the nurse or case manager, as early as the first day: Is my parent admitted as an inpatient, or under observation? Ask for the answer in writing, and ask again if anything changes. Hospitals must give Medicare patients held under observation for more than a day a written notice explaining the status and its consequences.

If your parent is under observation and is likely to need rehab, ask the attending physician whether inpatient admission is medically appropriate. Doctors make that call, and families can ask them to explain it.

What are the options if rehab will not be covered?

If Medicare will not cover a skilled nursing stay, the options are paying privately for rehab, going home with Medicare-covered home health therapy if the person is homebound and qualifies, or, for people with Medicaid, using Medicaid coverage. Ask the case manager to compare these for your parent's situation before discharge.

Home health physical therapy can be a good fit for someone who is safe at home with family support; it is intermittent, so plan for help between visits.

What should families do the day before discharge?

Confirm the status one more time, get the discharge plan and medication list in writing, and make sure the receiving rehab facility or home health agency has the orders. If your parent is going home with Medicare home health, ask when the first nurse or therapist visit will happen and who to call if it does not. If rehab is being paid privately because of observation status, ask the facility for its daily rate and what is included in writing before the transfer.

Also ask whether a Medicaid Long-Term Services and Supports screening should be done before discharge. If a longer stay or Medicaid-funded care at home is likely later, completing it in the hospital can save weeks.

Does observation status affect hospital bills too?

Yes. Under observation, hospital services are billed as outpatient care under Medicare Part B, which usually means copayments for individual services, and self-administered medications given in the hospital may not be covered. Ask the billing office for an estimate if a stay is lengthening, and keep the observation notice with your records in case questions arise later.

Medicare Advantage plans handle observation stays under their own cost-sharing rules, so members should call the plan as well.

Who can help if a Richmond family is confused about coverage?

Virginia's free State Health Insurance Assistance Program, known as VICAP, offers Medicare counseling through local Area Agencies on Aging, including Senior Connections - The Span Center on the Richmond side and the Crater District in the Tri-Cities. Counselors can explain observation status, Medicare Advantage rules and appeal options without selling anything. The hospital's case manager and patient financial services office are also obliged to answer questions about status and billing.

How do I choose a rehab facility near Richmond once coverage is clear?

Ask the hospital for the list of skilled nursing facilities that have accepted the referral, then check each one's CMS ratings, health inspection results and staffing on Medicare Care Compare or in our nursing home directory, which covers the Richmond region and the Tri-Cities. In the October 2026 CMS data, overall ratings among certified homes in our twelve localities ranged from one to five stars, so the choice matters.

Visit if you can, or call and ask about therapy hours per day, weekend therapy and how discharge planning works at the end of the stay.
